Steps to Licensure

LSW Exam Information: Testing is no longer required for the LSW. For information on the application process go to this page at the IDFPR website.
The Certification of Education form is a part of the application. The two-page form must be completed by JACSW to confirm your degree. Please refer to the link above with any questions about the application process.

Download the application and carefully read the instructions.
Certification of Education (ED) Form Process
Please keep in mind that forms are being processed monthly.
Submit the 2 page Certification of Education (ED) Form, page 17-18 of the application package, by doing the following:
1) Make sure the top portion (applicant section ONLY) and the right side edge on the second page is completely filled in.
2) Mail the form AND a self-addressed stamped envelope to:

3) The form will be returned to you using your self-addressed stamp envelope as soon as it is completed and approved by the Associate Dean to be included with your application.
Remainder of the Application
Per application instructions, submit your application and fee payment to the Illinois Department of Financial and Professional Regulation (IDFPR).
Once the IDFPR processes your application they will send you a letter with information on how to proceed forward.
